Pancreaticojejunostomy: images of an invagination technique.

چکیده
Pancreaticoduodenectomy is the treatment of choice for resectable periampullary tumors. Anastomosis between the remnant of the pancreas and the intestine can be fashioned by various methods. An anastomotic leak can result in sepsis and even death. The rate of these complications is reported to be 10-28.5% [1]. Two widely used techniques for fashioning a pancreatic anastomosis are practiced: end to side duct to mucosa anastomosis or telescopic invagination of the pancreas into the lumen of the bowel. Some authors have suggested that the invagination technique reduces the rate of anastomotic leaks and the rate of pancreatic fistula formation [2]. These complications directly correlate with morbidity and mortality after a Whipple procedure [3]. We would like to share with the readers interesting computerized tomography (CT) images of a pancreaticojejunostomy fashioned by a telescopic invagination anastomosis between the remnant of the pancreas and a loop of the small bowel.

JEJUNAL EVERSION MUCOSECTOMY AND INVAGINATION: AN INNOVATIVE TECHNIQUE FOR THE END TO END PANCREATICOJEJUNOSTOMY
ABSTRACT Background: The pancreatojejunostomy has notoriously been known to carry a high rate of operative complications, morbidity and mortality, mainly due to anastomotic leak and ensuing septic complications. Objective: In order to decrease anastomotic leak and its attendant morbidity and mortality in operations requiring a pancreato-jejunal anastomosis, and also in order to simplify the op...
متن کاملDuct-to-mucosa versus invagination pancreaticojejunostomy after pancreaticoduodenectomy: a meta-analysis
OBJECTIVE We aimed to compare the two most commonly used pancreatico-jejunostomy reconstruction techniques-duct-to-mucosa and invagination. METHODS Databases, including MEDLINE, EMBASE, Cochrane Library, and several clinical trial registration centers were searched. Randomized controlled trials that compared duct-to-mucosa and invagination pancreaticojejunostomy techniques after pancreaticodu...
متن کاملA Comparison of Two Invagination Techniques for Pancreatojejunostomy after Pancreatoduodenectomy
Background. The aim of the study was to compare two invagination techniques for pancreatojejunostomy after pancreatoduodenectomy. Methods. For effective prevention of the development of pancreatic leakage, we modified invagination technique that we term the "serous touch." We analysed the diameter of the main pancreatic duct, the texture of the remnant pancreas, the method of the reconstruction...
متن کاملReconstruction method after pancreaticoduodenectomy. Idea to prevent serious complications.
Pancreatic fistula after pancreaticoduodenectomy represents a critical trigger of potentially life-threatening complications and is also associated with markedly prolonged hospitalization. Many arguments have been proposed for the method to anastomosis the pancreatic stump with the gastrointestinal tract, such as invagination vs. duct-to-mucosa, Billroth I (Imanaga) vs. Billroth II (Whipple and...
متن کاملPancreaticojejuno Anastomosis after Pancreaticoduodenectomy: Brief Pathophysiological Considerations for a Rational Surgical Choice
Introduction. The best pancreatic anastomosis technique after pancreaticoduodenectomy (PD) is still debated. Pancreatic fistula (PF) is the most important complication but is also related to postoperative bleedings and pancreatic remnant involution. We support pancreaticojejuno anastomosis (PJ) advantages describing our technique with brief technical considerations. Materials and Methods. 89 co...
